Black Belt Test

 

DATE:  SATURDAY 7th  NOVEMBER 2009

INFORMATION:

All application forms must be with the administration by the 2nd of November for approval

After this deadline date applications will be rejected. 

This is to ensure certificates can be presented on the day.

 

Registration:

9.30am  - Peewee, Junior & Adults

 

 VENUE:   SPORTS CONNEXION, RYTON ON DUNSMORE, Main Hall

COVENTRY CV8 3FL  (off A45 south)

 

 THE NEW A’ APPLICATION FORM MUST BE DOWNLOADED FROM OUR WEB SITE, OLD FORMS

WILL NOT BE ACCEPTED UNDER ANY CIRCUMSTANCES

INSTRUCTORS make sure that all applicants photocopy their insurance licence card & attach it to their black belt application form “A”.    Applicants will not be taking the test without this proof of insurance.

Tick ABSENT at last test” or FAILED at last test” on the new A'application forms if this is relevant.

 

Over 17yrs *Photo copies* of A’& B’ course certs / Umpire course certs / Seminar certs

Jee-Goo attendance cards must be attached to black belt application form “A”

 

Clarification of the above will be checked against the association database.

 

A black belt certificate will not be presented on the day to any adult (over 17yrs) who pass the test who has not proved they have attended both “A & B” instructor courses.  At a later date upon attending the next “A & B” courses their black belt certificate will be presented on that day. Inform admin when you intend to attend in advance of courses

 

Application for 1st degree test fill in forms A / B Pee-wee, Juniors & adults.  Bring thesis on the day.

 

Application for senior 2nd & 3rd deg test fill in forms A / C,  form C’ - must meet the criteria to take test along with revelant copy proof of certs ect to take test..      

 

Application for adult half - degree tests fill in forms A & C,  form C’  you need to fill in along with revelant copy proof of certs ect to take test.

 

Application for junior / pee-wee star test’s fill in form A.

 

Application for International 4thdegree & above test fill in forms A / C / D (executive committee only )   

 

Payments on the day;

 

All applicants please bring correct fee  in an envelope no passport photo’s required.

 

National Certificate:  Under 17 yrs old for 1st Deg - £45.00

 

National Certificate:  Over 17 yrs old for 1st Deg - £55.00

                                                   

                                                    1/half - £65.00 - (First payment)

                                                 2nd Deg - £65.00 - (Second payment)

                                                    2/half - £78.00 - (First payment)

                                                 3rd Deg - £78.00  - (Second payment)

                                              3/half - £105.00 - (First payment)

                          Star Certification: Under 17 years old - £30.00 (Part Payment Towards)

 
First re-test through failure is “free of charge”.
 

Pee-Wee & Junior star grades take place each year that follows either a 1st or 2nd degree tests.

(Full details on star grades have been previously sent to all instructors)

 

An association embroided black belt & lapel pin will be presented on the day to all successful  

                   applicants along with their black belt certificate.

 

Instructors please ensure that all your black tags & black belts enter these grading exams.  You must represent your students on the day or appoint your assistant instructor to be present, and ensure your students fulfill all requirements above for this grading examination to save disappointment, thank you.
